- Residency Affidavit
___ 2. Birth Certificate of Student (Proof of age)

___ 3. Parent/Guardian Picture Identification

___4. Georgia Certificate of Immunization and Hearing Vision and Dental screening. (Form 3321 & 3300)
Certificado de vacunas (332i), Dientes, Vistas, y O{dos (3300) de Georgia.
___ 5. Withdrawal papers and las report card from previous School
Conditional placement will be made pending receipt of required Academics records.
Los documentas y ultimo reporte de notas de la escuela anterior.
